If you cannot complete the entire questionnaire at one sitting close the Browser. Your responses will be automatically saved. You can then complete the remainder of the questionnaire at the time of your visit or call the office (516-487-5455) for a Username and Password that will give you access to your questionnaire for a 24 hour period of time.
If you are unable to complete the online forms please download and complete one of the following forms which is most consistent with the reason for your visit. If you are unsure which one to complete use the Male Reproduction Questionnaire or call us (516-487-2700) for our advice. The completed questionnaire should be bought with you to your appointment.
